New Credit Card Policy

As of October 1, 2024 all patients of Neighborhood Naturopathic & Primary Care will be required to have a credit card on file in our secure electronic health record, Charm.

This policy will enable us to collect charges in a more timely manner, which is necessary to support our small business. Cards will be charged automatically for any no show or late cancellation fees. All other charges, such as deductibles, co-pays, and co-insurance, will be sent via invoice by Charm. Cards will be charged for any invoices not paid within 30 days. We are happy to keep HSA/FSA cards on file for these charges (though a credit card is also required), and can always accommodate payment plans for larger balances, if needed.

What’s next?

All patients will be receiving a new credit card authorization form - please take a moment to sign this new form in your Charm PHR account, under “Questionnaires”. Additionally, if not already on file, you will be asked to provide credit card information next time you are in clinic. Alternatively, you can add or update this information yourself.
