Services we offer for your patients
B-12 Shots
We use the methylcobalamin form of B-12, which for some is easier to utilize. We can provide monthly shots, but if your patient requires more frequent shots, please fax us a prescription to the fax number at the bottom of this page. Your patient will be contacted to schedule their B-12 shot. Please inform your patient there is a $30 charge not billable to insurance for this service.
Phlebotomy
We accept LabCorp and specialty lab kits at this time. Please have your patient bring in the printed requisition, any lab kits and their insurance card. Your patient will be contacted to schedule their blood draw. Please inform your patient there is a $30-40 charge for all blood draws that is not billable to insurance and the fee is variable depending on the complexity of the draw.

If you are referring your patient to us for primary care or specialty services to be established as a new patient, please fill out the form below and indicate this in the “Reason for Referral” section of the form.
